Brisbane North HealthPathways

The HealthPathways program is a joint project between Brisbane North PHN and Metro North Health, supported by Queensland Health.

HealthPathways is designed to provide our region's clinicians with up-to-date evidence-based patient assessment and management, referral and service information in an easy-to-access web and mobile-friendly format. The HealthPathways website contains state-wide Clinical Prioritisation Criteria (CPC) information and integrates with GP Smart Referrals, ensuring faster, streamlined processing of referrals within Queensland public hospitals.

The site continues to expand with the addition of new pathways supporting primary care and responding to emergent needs, such as the COVID-19 suite of pathways.

Access HealthPathways

Brisbane North HealthPathways has launched personal accounts as of Tuesday 18 November. The generic login details (Username: Brisbane, Password: North) has been retired and bookmarked generic logins will no longer work (they will redirect to the registration page). The stronger security will allow new features over time and improve user experience.

Available new features are the CPD reporting function which automatically log your HealthPathways usage and creates a report for submission to your CPD provider, and the Smart Search function which utilise AI to refine queries and present the most relevant results, reducing the need for extensive navigation.

Users must create a personal account to continue to access Brisbane North HealthPathways. Registration is easy and takes under 2 minutes.

See the FAQs for further information about personal accounts.

Register or sign in now and check out the new CPD and Smart Search features. Watch the video for assistance through the registration process.

Once registered, you can also request a demonstration in your practice by contacting the Integrated care team by phone on 07 3630 7300 or email HealthPathways@brisbanenorthphn.org.au
