Brisbane North PHN's Primary Care team enable high performance primary care by supporting practices across Brisbane’s northern suburbs, Moreton Bay, parts of the Somerset Region and Norfolk Island.
Primary Care team
Our Primary Care team can support you further with tailored support for your practice. We offer a range of programs and services for general practice, allied health professionals and pharmacists to enhance the quality and timeliness of care, and build your capacity to provide quality care. We also offer a range of digital publications for health providers in our region, including Practice Link for general practice staff and GP Link for clinicians.
Our areas of focus
- Digital health: digital readiness and supporting the use of key digital health tools.
- Immunisation support: boosting local coverage and delivering targeted alerts.
- Primary care reform: preparing for and implementing major reform initiatives.
- Practice accreditation: supporting practices to meet accreditation requirements.
- Continuous quality improvement: providing coaching, resources, project opportunities and clinical tools to drive quality improvement.
- Workforce development: offering and connecting GPs and practice staff with training, education, networking events and commissioning opportunities.

Practice Support
Alongside our Practice Support website and Practice Link newsletter, our practice support team is available to assist all general practitioners, practice staff, managers, and owners via phone and email.
Our support co-ordination engagement officer is available to answer your questions via the Practice Support phone number and email.
If you require specialised or extended support, we will connect you with the relevant team member who is experienced in your area of need.

General Practice Liaison Officers (GPLO)
A partnership between Metro North Health & Brisbane North PHN
The GPLOs are an experienced team of GPs who bring their unique insights to improving system integration between hospital and primary care services.
GPLO activities
GPLO activities include:
- practice visits to support the use of integrated systems including Health Provider Portal, GP referrals and HealthPathways
- working to improve discharge from hospital and clinical handover
- providing primary care advice to key working groups and contributing to the development of pathways of care, state-wide referral guidelines and action plans
- designing and implementing shared care models
- providing GPs with regular communications through the PHN’s monthly newsletter
- leading GP education events.
