Bridge to better Health Project
The Bridge to Better Health project is dedicated to enhancing the health care experience for patients with intellectual disabilities by boosting the capacity of practice nurses to implement annual health assessments in primary healthcare settings.
Evidence tells us annual health assessments work for people with intellectual disability, positively impacting health promotion, disease prevention and case-findings, but we know they require dedicated time to do well. We want to make Annual Health Assessments for people with intellectual disability work better for everyone involved, you, your patients, and the practice.
Who can take part?
Practices in South-East Queensland who:
- Have a practice nurse/s
- Can sign a data sharing agreement
- Identify approx. 3 patients with intellectual disability
Practice nurses who:
- Work in practice signed up to the study
- Are over 18 years
- Sign a consent form
Patients with intellectual disability who:
- See their GP at a practice signed up to the study
- Are over 15 years
- Sign a consent form (or where appropriate, have a substitute decision maker provide consent)
Advantages for you and your practice
- Takes a load off you, allowing you to see other patients while the nurse works on the annual health assessment
- More efficient for the practice
- Improves the health of people with intellectual disability in the practice
- Gives a clear health action guide
