Enhanced mental health eReferral pathway
Brisbane North PHN is strengthening the way mental health referrals are managed by moving to an enhanced central intake model that improves service navigation and supports more coordinated access to care.
The enhanced model aligns with Our Approach to Wellbeing and is designed to help ensure patients are connected to the most appropriate service based on need, rather than relying on direct referral pathways alone.
What is changing
Brisbane North PHN is transitioning away from the current rediCASE referral pathway as part of this enhanced model. The rediCASE referral URL and GP Integrator will be disabled on
30 September 2026. GPs will need to use the HealthLink SmartForm embedded in your practice software or secure webform as the referral pathway moving forward.
Why this model is better for General Practice
HealthLink SmartForms are secure, trackable digital referral forms embedded in GP practice software. Referrals are sent directly to the Medicare Mental Health intake team for initial assessment and referral, helping patients access the right support sooner. In our region Medicare Mental Health’s intake team is co-located with MH CALL. This collaboration with Metro North Mental Health strengthens the no wrong door approach, supporting more seamless access to care, clearer navigation and improved coordination across the mental health system.
Why use it
- More support around the patient – your referral can activate a broader care team and connect patients with services that may otherwise be difficult to identify or access.
- Better matching to need – central intake supports a more consistent approach to assessment and referral, helping patients get to the most appropriate service.
- Reduced fragmentation – the enhanced model is designed to reduce redirections, gaps in care and duplication across the system.
- Less pressure on GPs – many GPs carry significant responsibility for patients with mental health needs. A dedicated intake and referral pathway helps share that care more effectively.
- Improved visibility and tracking – HealthLink SmartForm provides a secure and
trackable referral process within clinical software. Faster connection to support – early referral through central intake can help patients access the right service sooner.
How to refer
- Open the patient record and click the HealthLink icon.
- Select Medicare Mental Health (1800 595 212) and your PHN, then press continue (top right-hand side)
- Review pre-populated details and attach documents.
- Submit the referral and track its status in your inbox.
If you are not using HealthLink-enabled software, you can still submit a referral using the secure webform.
