My Health for Life Fax Transmission Issue – Important Update for Healthcare Professionals

Diabetes Australia regret to inform healthcare professionals of an issue with the My health for life fax number (07 3506 0909) that has been ongoing since December 2024. The Diabetes Australia IT team has confirmed that faxes have only been intermittently received, and unfortunately, they have been unable to recover any missing referral documents that may have been sent.

We understand that many of you use fax to send referrals, and we are concerned that some referrals may not have reached the My health for life team. To ensure all referrals are received moving forward, we kindly ask that you:

🔹 Use Medical Objects (My health for life FM4064000RY) as the preferred method for sending referrals securely.
🔹 When sending referrals via fax to 07 3506 0909 please ensure you receive a notification that the fax has been received.
