
Mater Multicultural Health Coordination Program
Did you know you can refer your patients to the Mater Multicultural Health Coordination Program?
Mater has been recurrently funded by Queensland Health to deliver this program to people of all age groups living in Brisbane, Logan and Ipswich regions, who are from a multicultural background and have complex health and/or social needs.
This is a no-cost service, and there are no Medicare or visa eligibility requirements.
The Multicultural Health Coordination Program (MHCP) offers both brief interventions and extended care coordination to enhance the health and health literacy of participants. The program seeks to strengthen a patient’s connection between their primary care provider and tertiary and/or community services, as well as increase their understanding of the health system.
The team includes clinical nurses, a social worker and program staff who work in partnership with the patient, the patient’s family and support system, their GP and other health and community services. The program routinely provides care coordination, family and carer support, provision of health literacy education (health conditions, medications, and health system navigation) and facilitating patient access to community support organisations, GPs and medical specialists.
Patients, practice staff and GPs are able to refer to this program. Additionally, there is the opportunity to have a collocating Multicultural Health Coordination Nurse in your General Practice. Practices in the Brisbane City Council area can have a nurse up to 1 day per week collocate in their practice to provide this service to patients of their practice.
